Submit Your Sales Tax Exemption Certificate
Sales tax exemption certificates may be submitted by email, mail, fax, or by using the form below.
If you feel your order qualifies for a Sales Tax Exception, contact us before submitting your order. We are unable to refund sales tax without a full order return.
To set up your online account for sales tax exemption, we require a copy of your Sales Tax Exemption Certificate with the Sales Tax Exempt ID number, billing/shipping address, and your school or organization name.
If you have not yet created a login, please do so before submitting your tax exemption form. Creating an account allows us to link your login information with your tax exempt status, ensuring your exemption is applied correctly to your orders.
Save time! Include your purchase order information and items list in the form below. Band Shoppe can only accept Purchase Orders from state-recognized schools or districts.
POs must include the following:
- PO number
- PO billing address
- Shipping address
- List of items you’re purchasing, including the product code, size, color, gender, the approved dollar amount (with shipping and handling charges)
- An authorized signature
