Purchase Order Submission
Purchase Orders may be submitted by email, mail, fax, or using the form below.
Band Shoppe can only accept Purchase Orders from state-recognized schools or districts. POs must include the following:
- PO number
- PO billing address
- Shipping address
- List of items you’re purchasing, including the product code, size, color, gender, the approved dollar amount (with shipping and handling charges)
- An authorized signature
Tax Exemptions
If you feel your order qualifies for a Sales Tax Exception, contact us before submitting your order. We are unable to refund sales tax without a full order return. To set up your online account for sales tax exemption, we require a copy of your Sales Tax Exemption Certificate with the Sales Tax Exempt ID number, billing/shipping address, and your school or organization name. If you haven’t created a Band Shoppe login, please do so prior to completing this form.
